  5. Demonstrates that practice in the home is informed and improved by considering and acting on:

  • Research and developments in relation to the ways in which the needs of people are best met; and

  • The use of monitoring and review systems to make continuous improvements in the quality of care provided in the home.

The Manager of the home champions the ethos of Longmoor Care Ltd, to promote staff to work as an effective team to meet our residents needs set out in the Statement of Purpose.

Finally through regular internal and external review driving continual improvement the continuity of high quality care delivered to each resident is assured.

Experienced and Supportive

We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We take the time to get to know you and develop an individualised care plan that fits each individuals specific needs.

The Manager monitors staff through regular supervisions and yearly appraisals.

In turn the Registered Manager will attend a formal monthly Supervision with the Regional Operations Director, to discuss the home, planning, review, monitoring and evaluation.

An independent visitor will complete an audit monthly on the home to ensure the 5 key questions are being met.

Care Planning

Where possible, each resident has a Transition Plan put in place before coming to the home. Longmoor Care Ltd will works alongside Placing Authority when a child is in transition to Adult Services. The home does not take any emergency placements. The key elements we focus on are:

  • Health
  • Education
  • Physical and Emotional Needs
  • Safeguarding and Promoting Welfare
  • Cultural, Religious and Language
  • Racial and Leisure Needs

Only after several assessments will it be decided if an individual is suited to the Home and compatible with other residents who are already residing there.

Upon induction into the home, each resident receive a Resident’s Guide in a format that they can understand and an allocated Key Worker.

All residents are under constant review as each individual develops.

Safeguarding

Longmoor Care Ltd utilises the expertise of our Service Manager Philip Brown who is also a fully qualified NFPS restraint techniques trainer. This includes conflict management methods that enable staff to de-escalate potential issues. His qualifications are kept on site.

Upon induction all staff will receive safeguarding training. Our Independent Visitor, Russell Davies from HRA, is also a fully qualified NFPS trainer who is duty bound to ensure the staff use and maintain the correct techniques to ensure the safeguarding of all residents.

Wellbeing

Health

We understand that the health needs of our residents is vitally important to their emotional, intellectual and social well-being. Upon referral, each person’s health needs will be assessed to confirm that our locations are the correct environment for that person and that the correct medical facilities are within the location if required. A Health Plan is created for each person with information gathered from their families, Professionals and the person themselves. All outcomes are recorded on these plans and reviewed as necessary. Staff encourage residents to take a proactive role in looking after their day to day health and well-being. Where residents have specific health needs or conditions, they are supported to manage these subjects to their capacity. When a resident needs additional health or support, staff work with the residents’ placing authority to enable proper and immediate access to any specialist medical, psychological or psychiatric support required.

Our Community

Longmoor Care keenly promotes our residents become a part of our local community by joining various clubs and activities to meet their cultural needs for self-development. The level of supervision required is monitored and reviewed via full and ongoing assessments.

Positive Relationships

We promote the development of our residents in the following areas:

  • Mutual respect and trust
  • Understanding about acceptable behaviour
  • Positive responses to others

All residents have a Positive Behaviour Risk Assessment in place which enables them to actively seek positive relationship with others. The organisation also has a Managed Behaviour Plan in place that runs alongside their individual risk assessments. If it is identified that a resident requires any therapeutic or extra behavioural support Longmoor Care Ltd will contact further professionals and setup a meeting.

The Plans created from these assessments help our staff to:

  • Meet each resident’s personalised needs
  • Help to socially develop our residents
  • Understand our residents’ behaviour
  • Help residents to understand their behaviour
  • Promote healthy relationships
  • Demonstrate 'Mutual Respect' and the benefits of it
  • Understand the reasons for behaviour
  • Implement de-escalation techniques
